Overview
Description
Provides assessment, planning, coordination, and advocacy services for patients who may be pregnant. Provides non-clinical educational information, wellness programs, and organizes all additional VNA health-related services for the obstetric patient. Tracks and documents effectiveness and outcomes for obstetric-based services at the VNA Health Center Clinic sites.
1. Assesses patients requesting verification of pregnancy status.
2. Conducts pregnancy testing.
3. Provides patients pregnancy test results and reviews VNA services to support patient.
4. Initiates benefit eligibility based upon income and program requirements, medical presumptive eligibility (MPE).
5. Schedules OB-GYN surgeries, processes maternal fetal medicine (MFM) referrals, sends prenatal records to delivering hospital, and processes transfer requests.
6. Uploads OB medical records to patient charts for assigned location.
7. Provides patient education materials.
8. Follows up on patients through the first prenatal appointment.
9. Documents in the patient electronic medical record (EMR) all instructions and follow-up contact.
10. Informs patients of available resources and assists the patient in making appointments as needed.
11. Evaluates effectiveness of health education, wellness screening reminders, and coordinates additional education and care as needed.
12. Maintains all patient care coordination records and reports.
13. Networks effectively and develops key contacts with community and government-based agencies.
14. Ensures phone calls and requests for information are handled promptly and courteously.
15. Ability to work all Health Center hours as needed.
16. Ability to work effectively and efficiently in the EMR.
17. Familiarizes and complies with all VNA policies and procedures.
18. Indicates an interest in personal growth and VNA mission/goals and participates in staff meetings/in-services.
19. Meets standards of behavior expectations.
20. Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.
21. Travels to other VNA health center sites for necessary training and team meetings.
22. Other duties as assigned.
Minimum Requirements:
1. High school diploma required. Some college preferred.
2. Bi-lingual in Spanish/English as follows: Reading - Required. Written - Required. Speaking - Required.
3. Effective organization and communication skills.
4. Community health experience preferred.
5. Experience working with pregnant women preferred.
6. Excellent computer skills required. Good working knowledge of Microsoft Office software, especially Word and Excel.
